How to use

A short welcome to HIMForward CareTrack. This is a fictional demo for closing the screening-to-care loop.

1. 1Choose a roleUse Switch role. There is no password. Coordinator, family physician, clinic, or programme manager.
2. 2Add or open a screening resultOpen a person from Patients, or add a result. The app does not diagnose.
3. 3Review the AI priority (the doctor decides)Read why the case is Priority 1, 2, or 3. A family physician accepts, changes, or rejects the suggestion.
4. 4Refer in network and bookSend a fictional referral. In-network clinics are listed first. Booking a visit is not completed care.
5. 5Remind and follow upReminders stay in this browser. If someone may not attend, record a barrier and a contact attempt.
6. 6Record the outcome and see the Loop Health ScoreMark care received and verified. The dashboard counts verified cases within 120 days of the result.
